Ultrasound Assessment of Internal Carotid Stenosis Pre- and Post-endarterectomy of Contralateral Near-occluded Carotid

چکیده

Internal carotid artery (ICA) stenosis often presents bilaterally and is a major cause of stroke. Carotid duplex ultrasound used for estimation via peak systolic velocity (PSV), end diastolic (EDV), ICA/common (CCA) ratio. endarterectomy (CEA) common surgery ICA carries high risk stroke mortality. In near occlusion with >90% stenosis, collateral flow through contralateral can compensate increased velocities, artificially elevating stenosis. We hypothesize that by estimating velocities the unoperated pre- post CEA near-occluded ICA, we better estimate artificial elevation ipsilateral A retrospective study was conducted on 239 patients, >18 years old, who underwent routine examination 2 at single institution. Qualified patients had bilateral occlusive disease one side CEA. Near determined showing or CTA interpreted physician. Preoperative to were recorded compared postoperatively 6 months. Student t test in these analyses. Post-CEA occluded PSV, EDV, ICA/CCA ratio significantly decreased (PSV pre-CEA = 145 ± 80 vs post-CEA 128 67; P < .001; EDV 42 28 36 230; 1.85 1.2 1.75 1.2; .032). Based new values if classify as <50%, there an 81.6% increase cases post-CEA. If 50% 69%, 72.2%. >70%, 26.7%. Finally, classifying 80% 99%, 100%. The are elevated presence innate occlusion, (Figure). This provides novel insight into revision criteria accurate avoid unnecessary high-risk surgery.

Carotid angioplasty and stenting in a patient with high grade stenosis of Internal Carotid Artery associated with both vertebral arteries and contralateral carotid occlusion

Severe internal carotid artery (ICA) stenosis is a common cause of cerebrovascular accident (CVA) in middle-aged patients. Contralateral carotid occlusion (CCO) in patients with severe ICA stenosis is associated with high risk of CVA. Carotid endarterectomy (CEA) is associated with more complications in patients with CCO than those without CCO. In this study, we present the case of a 61-year-ol...

Doppler ultrasound assessment of the internal carotid artery following carotid endarterectomy.

Doppler-shifted ultrasound with spectral analysis was used to assess the internal carotid arteries of 48 patients who had undergone carotid endarterectomy (58 carotid endarterectomies). Three patients died in the immediate post-operative period, therefore 45 patients, who represented 54 endarterectomies, were assessed for up to six years following surgery (mean--thirty-four months). Ultrasound ...

Occluded but nonthrombosed internal carotid artery: an indication for endarterectomy.

The indications for carotid endarterectomy are sometimes imprecise. Total occlusion of the internal carotid artery has often implied irreversibility. Carotid thromboendarterectomy was performed on eight patients who on angiography had a complete occlusion of proximal internal carotid artery but showed antegrade opacification of the infraophthalmic carotid siphon. Direct magnification, delayed r...

Overestimation of contralateral internal carotid artery stenosis before ipsilateral surgical endarterectomy.

OBJECTIVE The aim of this study was to investigate a possible overestimation of the degree of contralateral carotid artery stenosis by duplex in patients with significant bilateral carotid stenoses who are to undergo carotid endarterectomy (CEA). METHODS A retrospective analysis was performed of all patients undergoing CEA in our center over a period of 11 years. Pre- and postoperative duplex...

Carotid Stenosis With Prophylactic Carotid Endarterectomy

Background and Purpose: The value of carotid endarterectomy in asymptomatic patients with high-grade stenosis is controversial. The objective of this study is to compare the immediate and long-term outcome of patients after carotid endarterectomy for asymptomatic carotid stenosis (>75%) with the reported natural history of patients followed nonoperatively to determine whether carotid endarterec...
